A scrotal wall abscess is the formation of an abscess within the scrotal wall.

Clinical presentation
Patients may present with an acute tenderness within scrotum usually with overlying erythema, fluctuant mass and sepsis.
Pathology
May occur from two mechanisms:
primary superficial infection of the hair follicles, sweat glands or skin abrasions
secondary infections from regional infections from the epididymis or testis, periuretheral phlegmon or extension from anorectal abscesses
rarely from distant causes such as perforated appendicitis 4

Usually initial and primary imaging modality of choice.
Hypoechoic collection in the scrotal wall with surrounding cellulitis. Intrinsically, there can presence of low level echoes or septations.
Treatment and prognosis
It is commonly managed with incision and drainage. Urology consult is recommended for intra scrotal involvment.
