A shepherd’s crook right coronary artery is an anatomic variant of the course of the proximal right coronary artery. While the RCA origin is normal (arising from the right coronary cusp), the variant is characterized by a tortuous and high course, usually in the proximal segment just after its origin.
Its prevalence is estimated at approximately 5%. While often not clinically significant, its presence may complicate percutaneous intervention 1.
