Dural venous sinus thrombosis

Presentation

Worsening headache.

Patient Data

Age: 45 years
Gender: Male
mri

Filling defect located in the torcula and right transverse sinus with T1 and T2 hyperintense signal, as well as signal drop off on SWI images. Findings are indicative of blood clot.

Giant arachnoid granulation (2 cm) in the superior sagittal sinus.

Case Discussion

Typical MRI imaging features of dural venous sinus thrombosis. The most sensitive MRI sequences without contrast injection for the detection of clot are susceptibility sequences (SWI or GRE). Whereas a T1-weighted sequence with contrast injection is the most sensitive and specific for the imaging diagnosis of dural venous sinus thrombosis.

Patients with untreated dural venous sinus thrombosis are at risk of developing infarction with the potential of hemorrhagic transformation.
