Presentation

Respiratory distress.

Patient Data

Age: 22 days
Gender: Female
x-ray

Hyperexpansion of the left upper lobe with a portion herniated across the midline. There is shift of the mediastinum to the right, flattening of the left hemidiaphragm and some compression of the left lower lobe.

Case Discussion

Features are those of congenital lobar emphysema of the left upper lobe.  This is the most commonly affected lobe (~40 to 45%).
