Epiploic appendagitis

Diagnosis almost certain

Presentation

Admitted to the emergency ward with a 3 days history of left lumbar pain. Physical examination revealed a low-grade fever and tenderness and defense with left lumbar localization; laboratory findings revealed a leukocytosis.

Mass lesion with fat density with a hyperdense rim, anterior to the descending colon.

Case Discussion

Abdominal computed tomography work up showed a mass lesion with fat density anteriorly adjacent to the descending colon, with a hyperdense rim and surrounding stranding.  

Antibiotic treatment commenced after radiological assessment of epiploic appendagitis. Clinical improvement was seen on the ninth day from the onset of symptoms.
