Case contributed by Shervin Sharifkashani
Diagnosis certain

Presentation

Facial trauma and suddenly swelled right side eyelids.

Patient Data

Age: 60 years
Gender: Female

Right orbital medial wall blow-out fracture with postseptal and pronounced eyelids emphysema. Foci of thick mucosa in some paranasal sinuses and enlarged cerebral ventricles are also seen.

Case Discussion

The case demonstrates the non-contrast MDCT feature of the pronounced eyelids emphysema due to minimally displaced orbital medial wall blow-out fracture. It denotes that there is not always a correlation between the severity of the fracture and the emphysema.
