Large lobulated soft tissue mass with irregular margin centred on the uterine cervix with obstruction of its lumen and distended fluid-filled uterine cavity (hydrometra). There is extension to the upper 2/3 of vagina with bilateral parametrial infiltration. It elicits an isosignal to the myometrium on T1, heterogeneous high signal on T2 with restricted diffusion and moderate heterogeneous enhancement on postcontrast sequences. The tumour invades the posterior wall of the urinary bladder well-demonstrated on T2 and postcontrast sequences. No significant pelvic lymphadenopathy.
No para-aortic lymphadenopathy or distant metastases on CT (not shown).
