There is an ovoid density projected over the right lower quadrant of the abdomen, suspicious for an appendicolith.

Case Discussion
Later the same day the patient underwent a laparoscopic appendectomy. Pathological review of the surgical specimen showed acute gangrenous appendicitis with rupture, appendix measuring 8 x 2.5 x 2 cm, with a 2 cm appendicolith.
