Massively enlarged cardiac silhouette with most severe involvement of the right atrium.

Massive enlargement of the right atrium and the right ventricle compressing the left atrium and left ventricle. Moderate pericardial effusion with a drain in place. A right ventricular mass (~2 cm) adherent to the interventricular septum may represent a thrombus. Trace pleural effusions and dependent atelectasis.
Case Discussion
This patient was known to have Ebstein anomaly, a rare congenital heart condition characterized by displacement of the tricuspid valve into the right ventricle, often with tricuspid regurgitation. The right atrium is typically enlarged, sometimes massively as in this case.
