Do you think this lesion is intramedullary or extramedullary? Why?
The lesion appears to be intramedullary on sagittal images, and a convincing 'claw sign' (the cord 'wrapping' around the lesion) is seen on axial lesions. The extent of oedema would also be unusual in an extramedullary lesion.
An enhancing intramedullary nodule at C7 is present with extensive perilesional oedema. It is an isolated lesion, with no evidence of leptomeningeal disease.
