Presentation
Chronic right lower abdominal pain with no fever.
Patient Data
Long appendix of subcecal location with thickening and enhancement of the appendiceal tip forming a small mass obliterating the appendiceal lumen with regional enlarged lymph nodes. No surrounding fat stranding or abscess formation.
Circumferential thickening of the colonic wall and terminal ileum, probably inflammatory bowel disease (colonoscopy was requested).
A simple cortical right renal cyst is noted.
Case Discussion
CT features of small enhancing mass at the appendiceal tip with enlarged regional lymph nodes.
The patient went on to have appendectomy with a histopathological exam that confirms the diagnosis of appendiceal carcinoid with metastatic regional lymph nodes.
Appendiceal carcinoids are neuroendocrine tumors that classically arise at the appendiceal tip from subepithelial neuroendocrine cells.
