Metastatic cecal adenocarcinoma

Diagnosis almost certain

Presentation

Work up for abdominal pain and chronic constipation.

Patient Data

Age: 65 years
Gender: Male

Increased wall thickness due to tumoral infiltration is present at the cecum and proximal of the ascending colon, accompanied by pericolic fat stranding and several regional enlarged lymph nodes with SAD less than 15 mm.

Multiple nodular opacities are seen at both lungs less than 25 mm. One of them shows central cavitation, located in the laterobasal segment of the right lower lobe.
A few patchy subpleural opacities are also seen at the right lower lobe.

Several small low-enhancing masses are seen at the liver less than 20 mm. Additionally, a few thin-walled non-enhanced hepatic cysts are scattered with a maximum diameter of 44 mm.

A 40 mm simple cortical cyst is noted in the mid portion of the left kidney.

The prostate gland is enlarged. 

Degenerative changes such as osteophytosis are seen in the lumbar spine.
Grade I spondylolisthesis of L4 on L5 is present. 

Case Discussion

Cecal mass; pathology proved adenocarcinoma with regional enlarged lymph nodes and pulmonary and hepatic metastases.
