Presentation

Cough and fever.

Patient Data

Age: 3 years
Gender: Male

Partial effacement of the right heart border silhouette due to adjacent consolidation in the right middle lobe.

Left lung clear.

Case Discussion

The loss of the right heart silhouette is indicative of right middle lobe pathology, typically pneumonia, as in this case of a pre-school child.

The silhouette sign principles described by Felson apply equally to children and adults.
