What poor prognostic indicators are present?
Large hernia with 1) compression of the ipsilateral lung and 2) severe mediastinal shift resulting in compression of the contralateral lung. The degree of pulmonary hypoplasia is the major predictor of mortality risk in CDH.
Name which lines and tubes are present. Are they positioned appropriately?
ETT, NGT, UAC & UVC in situ. UAC at T6 level (T6-T10 appropriate). UVC location difficult to assess given the altered anatomy, but is probably in the right atrium or atriocaval junction (atriocaval juntion is the ideal position). ETT, NGT in good position.
Multiple bowel loops fill the right hemithorax, causing mediastinal shift to the left.
