Renal vein thrombosis

Diagnosis probable

Presentation

Acute onset pain in right lumbar region.

Patient Data

Age: 40 years
Gender: Female
mri

Markedly elevated serum creatinine level precluded administration of contrast. MR imaging (without contrast) was planned. 

Markedly swollen, enlarged right kidney, with T2 hypointense parenchymal signal, loss of corticomedullary differentiation, and pericapsular heterogeneity. 
T2 isointense thrombus is seen within the right renal vein. The left renal vein is normal.

Case Discussion

MRI may help in the diagnosis of renal vein thrombosis as creatinine is frequently elevated precluding the contrast-enhanced venogram many times.
