The MRI sequences demonstrate:
on the left: loss of soft tissue thickness with deep ulcer cavity of the buttock extending up to the ischial tuberosity which appears thickened of low signal on both T1 and T2 with cortical erosions (osteomyelitis). Associated oedematous infiltration with enhancement surrounding the ulcer cavity
on the right: loss of soft tissue thickness with deep ulcer cavity extending up to the ischial tuberosity which shows a normal cortical thickness and bone marrow signal
atrophied gluteal muscles with fatty degeneration
reactive left inguinal lymphadenopathy

right hydrocele is noted
