A large intensely enhancing highly vascular soft tissue lesion is noted in the region of posterior choana and nasopharynx compromising the nasopharyngeal airway. The lesion is extending to the left pterygopalatine fossa with expansion and remodeling of the fossa, and with anterior bowing of the posterolateral wall of left maxillary sinus. Left maxillary sinus is almost obliterated and opacified.
From pterygopalatine fossa, the lesion is extending through pterygomaxillary fissure to the left infratemporal fossa and masticator space. The lesion is also extending through the left inferior orbital fissure into the orbit with expansion of the fissure.
There is intracranial extension into the left middle cranial fossa through the widened superior orbital fissure. Left cavernous sinus and temporal lobe are compressed, thereby causing engorgement left superior ophthalmic vein. Anteriorly the lesion is extending to the left nasal cavity with expansion remodeling of the bony nasal septum.
