Case contributed by Yi-Jin Kuok
Diagnosis certain

Presentation

Long tract signs.

mri

Canal stenosis worse at C3/4 due to posterior osteophyte disc complex. Associated spinal cord edema at C3/4 extending inferiorly two segments.

x-ray

Anterolisthesis of C4 with respect to C5 on flexion secondary to degenerative facet joints. This likely contributes to canal stenosis at C4/5

Case Discussion

On MRI imaging alone it is difficult to appreciate the facet joint instability at C4/5. This case demonstrates the value of functional imaging.
