Sagittal T1 & T2 MRI of the spine shows a dorso-lumbar syrinx with herniated cerebellar tonsils, in a patient with known Chiari I malformation. Although more than 20 % of patients with a Chiari I malformation have an associated syrinx, they usually are located in the cervical spine. This case demonstrates a dorsolumbar syrinx associated with Chiari I.
A ventriculus terminalis would present like this, except less voluminous, and located distally (as the name says it, at the filum terminalis).
