On contrast-enhanced lumbar spine MRI, a lesion was observed in the spinal canal - extradurally at the L4-5 level, with a signal similar to subcutaneous fat across pulse sequences, showing high intensity on T1W and T2W, and low intensity on T1FS and STIR, non-enhancing after contrast injection. The lesion compresses the dura, creating a Y-shaped appearance.
In addition, degenerative disc-vertebral lesions, Modic II changes, Schmorl's nodes, and multi-level posterior disc herniations were also noted.
