The MRI findings are characteristic of a lipoma in the quadrigeminal cistern. In the absence of fat-saturated sequences, the presence of chemical shift artifact on GRE will be helpful in confirming that the lesion contains fat. This is a case of incidental finding, with no clinical manifestations, so no further intervention is required.
