Left hip pain.
Patient Data
Frontal radiograph of the pelvis and hips reveals a short broad femoral head (black arrow) and a relative shortening of the left limb (compare levels of lesser trochanters (yellow arrows) in keeping with a coxa magna deformity in this patient with know Legg-Calve-Perthes (LCP) as a child.
Case Discussion
Coxa magna is a result of insult to the femoral head or epiphysis in childhood. Coxa magna deformity is a late radiographic finding in this patient with Legg-Calve-Perthes. Patients with Legg-Calve-Perthes is at risk for developing early osteoarthritis (OA) and femoral acetabular impingement (FAI).
Differential Diagnosis :
- developmental dysplasia hip (DDH) - In DDH, the acetabulum is shallow.
- slipped capital femoral epiphysis (SCFE) - In SCFE, femoral head slips medially & posteriorly with a normal acetabulum.
- Legg-Calve-Perthes (LCP) - In LCP the acetabulum is normal.
