Presentation

Lumbar pain and bilateral leg pain.

Moderate central lumbar canal stenosis from L3 to L4/5, with mild degenerative endplate signal changes. The lumbar canal stenosis is confirmed on the T2 imaging and is maximal from L3/4 to L4/5 and was consistent with the clinical presentation. Note the thickened and hypodense dorsal dura, and correlate with subsequent post-operative CT imaging.

Diffuse calcification of the spinal dura mater from thoracic to lumbar spine.  Note that there is no suggestion of calcification of cauda equina or nerve roots. Ligamentum flavum is not involved.

L3-4 laminectomy defect, some gas density, Day 2 post-surgery.

Case Discussion

The imaging shows calcification of the dura, with no intrathecal calcified structures, excluding arachnoiditis ossificans, and without involvement of the ligamentum flavum. One of the recognized underlying causes is chronic renal failure, either alone or with associated hyperparathyroidism. The patient presented with symptomatic lumbar canal stenosis, a laminectomy was performed, and the dural calcification was an incidental finding on post-operative imaging.  There was only mild renal impairment and no other cause identified.
