MVA, incidental findings during trauma series workup

Patient Data

Age: 32
Gender: Male

Multi-focal infiltrates in the upper lobes.

Widening of the right paratracheal space.

Large right paratracheal nodal mass measuring 40 mm in diameter displacing the SVC anteriorly. Multiple smaller (15mm) superior right mediastinal and pre vascular nodes are visible as well as a large 20 mm subcarinal node. 13 mm right supraclavicular lymph node. No hilar adenopathy.

Multiple spiculated peripheral peribronchial nodular opacities in upper lobe predominant distribution, ranging in size from 5 mm to 20 mm. Several of these appear confluent. The pleural surfaces are clear. Following No thoracic spinal lesions.

No rib or thoracic spine fracture identified.

Case Discussion

Biopsy proven SARCOID: The smears are hypercellular with scattered clusters of epithelioid cells forming granulomas in a background of mixed lymphoid cells, bronchial cell, degenerate granulomas and blood.
