Bucket handle tear - medial meniscus

Case contributed by Ahmed Elowaidy
Diagnosis certain

Presentation

Knee pain after knee twisting.

Patient Data

Age: 30 years
Gender: Male

The sagittal images show a linear high signal within the posterior horn of the medial meniscus reaching to the inferior articular surface and meniscocapsular attachment suggestive of meniscal tear.

There is absent bow tie on 2 consecutive images, with a low signal structure that is seen in the intercondylar notch coursing inferior to the PCL (Double PCL sign) and can be traced to the medial meniscus, this suggests bucket handle tear of the medial meniscus.

Case Discussion

The coronal images clearly confirm the diagnosis, and axial images beautifully demonstrate bucket handle tear of the medial meniscus.
