Hyperstimulated ovarian torsion

Presentation

Severe right abdominal pain following ovarian stimulation for infertility.

Patient Data

Age: 30 years
Gender: Female
ultrasound

Massively enlarged right ovary (~1200 mL) containing multiple cysts (follicles) with very poor blood flow on color Doppler. The less enlarged left ovary contains multiple large follicles and demonstrates blood flow. 

Free intraperitoneal fluid around the spleen.

Case Discussion

Appearances are consistent with hyperstimulated ovaries and torsion of the right causing infarction.
