Presentation

HIV positive patient complained of dizziness, confusion and behavioral changes.

Patient Data

Age: 45 years
Gender: Male

Bilateral deep white matter altered signals that display low signal in T1, bright signal in T2 and FLAIR without significant enhancement after contrast administration. No diffusion restriction on DWI. The signal alteration also involves the brain stem and cerebellar peduncles.

Case Discussion

Progressive multifocal leukoencephalopathy is a demyelinating disease that results from infection by JC virus in immunocompromised patient. The prognosis is poor especially when untreated.
