Erosion of the odontoid peg can result from a number of causes:
- inflammatory arthropathy
- crystal arthropathy
- calcium pyrophosphate arthropathy (CPPD): relatively common
- gout
- non-inflammatory arthropathy: osteoarthritis
- infective arthropathy

- metastases or primary bone tumor: rare
