There is a medium sized expansile lytic lesion involving the expected location of the right geniculate ganglion of the facial nerve. It gives the classic "honeycombing appearance " of the facial nerve venous malformation (hemangioma). There is sparing of the otic capsule. Almost all the course of tympanic segment of the facial nerve is involved. It also extends along the labyrinthine segment of the facial nerve reaching the anterior wall of the IAC with associated bone destruction.
