Epidural hematoma - thoracic and lumbar spine

Presentation

Limb weakness.

Patient Data

Age: 70 years
Gender: Male

Large epidural collection causes severe narrowing of spinal canal and marked mass effect on spinal cord between C6 and L3. The collection has high T1 and low T2 signal with no discernible post contrast enhancement. Hematoma is considered the most likely diagnosis with characteristics not typical for abscess or malignant process.

Note is also made of left lung mass with multi-focal bone metastases. A pathological fracture is present at the L3 vertebral body with minor retropulsion into the spinal canal. This contributes to the severity of canal narrowing at this level.

The cause of the patient's epidural hematoma was not certain, however the fracture at the L3 level may have contributed to this.
