Primary cutaneous melanoma of foot

Diagnosis almost certain

Presentation

Nodule involving sole of the foot for 1 year.

Patient Data

Age: 20 years
Gender: Male

A soft tissue density is noted involving sole. No calcification.

No bony abnormality.

Biopsy of the lesion was done.  2 differencials - Anaplastic ca, amelanotic melanoma. 

Referred for chest radiograph and ultrasound abdomen - pelvis and inguinal regions.

Both lung fields are clear.

No abnormality of soft tissue or bony thorax.

LEFT CALF

Three subcutaneous lesions are noted

  • two hypoechoic nodules seen lateral to short saphenous vein - 4 - 5 mm
  • hypoechoic nodules seen medial to short saphenous vein - 3 mm  

LEFT popliteal fossa        

An ovoid lesion   21 x 17 mm with loss of hilar fat and central vascularity.

LEFT inguinal region

Three nodes/lesions are noted. The largest lesion/node is measuring 19 x 16 mm with loss of hilar fat and central vascularity.

Another node with asymmetric cortical thickening.

Abdomen and pelvis

No hepatic lesions.

Case Discussion

Excision of subcutaneous lesions in calf, popliteal nodes and inguinal nodes was done.

Histopathology - Anaplastic ca  / melanoma.

IHC study  - Melanoma.
