Splenic infarction in case of malaria

Diagnosis almost certain

Presentation

Complaining of headache for 1 day followed by 5 days of fever. Complaining of abdominal pain for the last two days.

Patient Data

Age: 14-15 yrs
Gender: Male
ultrasound

The spleen is mildly enlarged. No calcification is noted. Multiple, geographic, hypoechoic, peripherally based lesions are noted in the spleen with loss of vascularity  The largest lesion measures up to 35 mm. The rest of the abdominal ultrasound was normal.

Case Discussion

Ultrasound findings suggest multiple splenic infarcts.

Lab: Plasmodium vivax - positive.

Splenic infarcts and splenic rupture are complications of malaria.
