Pre-and post-contrast study.
Enhancing left cerebellopontine angle mass with broad dural base along the petrous ridge measures approximately 2.4 x 1.3 cm transaxially and is most consistent with a meningioma.
Minor adjacent dural tail to the anterior margin of the tentorium cerebelli and along the petroclinoid ligament.
No extension into the internal acoustic canal. No adjacent hyperostosis.
Mild lateral displacement and distortion of the pons and mild crowding of foramen Magnum without tonsillar herniation.
No other intra or extraaxial mass nor abnormal enhancement.
Conclusion: Left CPA/petrous ridge meningioma.
The left cerebellopontine angle solid mass is again demonstrated with the same size, showing to protrude into the internal acoustic canal, expressing iso-intensity to the adjacent cortex on T1, high signal intensity on T2 and homogeneous and vivid contrast-enhancement. The features are strongly suggestive for a meningioma, the dural tail makes a schwannoma a less likely differential.
MICROSCOPIC DESCRIPTION:
Paraffin sections show a moderately hypercellular meningioma with a well developed syncytial architecture. Tumor cells have uniform nuclear features. No mitotic figures are identified and there is no necrosis. No evidence of brain invasion is seen. Broad attachment to dura is identified.
DIAGNOSIS:
CP angle tumor: Meningioma (WHO Grade I)
