Double PCL sign.

Tear of anterior horn of the medial meniscus.

ACL tear - likely chronic.

Patellofemoral chondromalacia.

Small joint effusion.

Case Discussion

The double PCL sign is visulised in the context of a severe meniscal injury.  The flipped meniscal fragment lying anterior to the native PCL giving the impression of two parallel PCLs.

It can occur in conjunction with other injuries, such as an ACL tear as in this case.
