Presentation

An adult male complains of severe abdominal pain and distension starting one day ago.

Patient Data

Age: 44 years
Gender: Male
x-ray

Multiple air fluid/levels are seen within dilated large bowel that suggest intestinal obstruction.

Barium

Gastrografin enema at the point of twist demonstrates a characteristic beaklike termination of contrast column or bird of prey sign. It suggests sigmoid volvulus.

Case Discussion

The sigmoid volvulus here was reduced during enema examination by turning the patient from side to side. Later on, a well lubricated rectal tube was passed through the point of obstruction under fluoroscopic guidance.
