Fibromatosis - dorsolumbar region

Diagnosis almost certain

Presentation

RIGHT lower dorsal paraspinal swelling with pain - - 2 - 3 wks. Radiograph - DL spine - No bony abnormality.

Patient Data

Age: about 22 yrs
Gender: Female
ultrasound

Well-defined ovoid lesion is seen in region of interest.

Size                 - Craniocaudal - 62 mm,  Axial - 50 mm, Anteroposterior - 20 mm

Location          - Deep to subcutanoeus fat , Superficial to ribs, Involves paraspinal muscle

                        NO adjacent rib cortex erosion.   

Morphology -   Hypoechoic.   NO calcification. Not compressible.

Dopplar           - Flow signals are noted.

Case Discussion

Well-defined margins with vascularity were two basic findings favoring solid lesion of non-specific etiology.  However, these two basic findings were ignored ( definitely by me ) and a suspicious finding of central necrosis was given more importance. 

MR done a wk later, showed solid tumor ( not uploaded, no copy right ).

Surgical excision was done - operative finding of a solid lesion.

Histopathology - no necrosis. Solid lesion reported to be fibromatosis.
