If the impact was on the right side of the head, how have the left sided contusions happened?
These are "contre-coup" injuries. This phenomenon happens in high energy head injuries where the brain tissue continues to travel in the direction of impact and stops against the skull at force, leading to bleeds.
Multiple internal and external cranial injuries.
There is a large right sided soft tissue swelling over the right temporal bone (sub-galeal haematoma).
Underlying this, there is an acute right subdural haemorrhage, as well as blood in the subarachnoid spaces.
There is mild mass effect, and evidence of haemorrhagic contusions in the left inferior temporal lobe.
