It doesn't appear that the patient has cirrhosis... what are some options for this mass before contrast administration?
The single mass is well-circumscribed without significant change to the surrounding parenchyma, so the differential shifts around a little in favor of more benign entities, but realistically, the differential is quite wide. Don't forget that, although less common, hepatocellular carcinoma can occur in a non-cirrhotic liver.
There is a single 4.5 cm mass in the right hepatic lobe:
- heterogeneous T2 signal intensity
- uniform T1 hypointensity
- no evidence of intracellular lipid on the IP/OP sequence (mild diffuse hepatic hypointensity on the IP sequence is artifactual)
- the mass demonstrates restricted diffusion
There is no evidence of cirrhosis or other diffuse hepatic disease. No other abnormality in the abdomen.
