Left vesico-ureteric junction calculus

Case contributed by Andrew Dixon
Diagnosis certain

Presentation

Left flank pain and hematuria

Patient Data

Age: 45
Gender: Male

There is mild left hydronephrosis and hydroureter.  Mild left perinephric and periureteric fat stranding.  There is a 3 mm calculus sitting at the left vesicoureteric junction. Despite the stone appearing to be largely within the bladder lumen, since the patient is scanned prone, the antidependent position of the stone implies it is not yet fully clear of the VUJ. The right renal tract is normal. Diffuse fatty infiltration of the liver.  Uncomplicated colonic diverticulosis.

Case Discussion

3 mm calculus sitting at the left vesicoureteric junction with associated mild hydroureter and hydronephrosis. This case demonstrates how scanning the patient in the prone position can assist in determining whether a stone has fully passed into the bladder. In this case, despite the stone appearing to be largely within the bladder lumen, the antidependent position of the stone implies it is not yet fully clear of the VUJ.
