Lunate dislocation and ulnar styloid fracture

Age: 25 years
Gender: Male

The lunate is triangular on the frontal projection and the lateral confirms volar dislocation in keeping with a lunate dislocation. There is also an undisplaced fracture the ulnar styloid process.

Dorsal backslab. Volar rotation and dislocation of the lunate into the carpal tunnel, with disruption of the scapholunate ligament. Minimally displaced transverse fracture through the ulnar styloid process. Alignment elsewhere is normal. No other fractures identified.

Case Discussion

Good example of a lunate dislocation, not to be confused with a perilunate dislocation. It represents the end (4th) stage of perilunate instability, where all the ligaments surrounding the lunate are disrupted. The lunate almost always dislocates in the volar direction.
