Right scrotal pain.
Patient Data
The right testis measures 5 x 3 x 3 cm. the testis is diffusely hypoechoic with heterogeneous echotexture and increased internal vascularity consistent with history of orchitis.
The right epididymal head demonstrates increased vascularity when compared to the left.
A large septated right sided scrotal collection is seen measuring 7 x 4 x 5 cm in diameter. It is predominantly anechoic with internal septations.
The left testis measures 5 x 4 x 3 cm. It demonstrates normal echotexture and echogenicity, with no focal lesions seen. The left epididymis is normal, with a small epididymal cyst. A small left sided hydrocele is noted.
Conclusion:
- Right sided epididymo-orchitis, with a large septated hydrocele. No evidence of abscess formation or testicular infarction.
- Small left sided hydrocele. The left testis is normal.
Case Discussion
Orchitis is a common cause of acute scrotal pain, much more than testicular torsion.
