Multiple T2 and FLAIR hyperintense lesions are seen in the brain stem and the medial cerebellar hemispheres. These lesions are T1 and DWI isointense with no significant post-contrast enhancement. No definite lesions are seen in the basal ganglia, thalami or cerebral white matter.

This is a case of neurofibromatosis; one of the phakomatosis (with cafe-au-lait skin lesions and axillary freckling) showing typical focal abnormal signal intensity (FASI) or unidentified bright objects (UBOs) lesions.
