Presentation

Left facial droop and ataxia

Patient Data

Age: 85 years
Gender: Male

Subtle wedge-shaped region of reduced attenuation in the left pons suspicious of acute ischemia which would correspond to the location of the facial nerve motor nucleus. No dense vessel sign (the basilar artery was normal on sagittal series and there was no thrombosis on CTA).

Mild deep and periventricular white mater hypodensity most likely represents chronic small vessel ischemia. No hemorrhage.
