Presentation
Married. Adopted child. Right groin mass. Initially thought to be a hernia.
Patient Data
Irregular solid-cystic mass in the right inguinal canal with morphology of a testis.
Smaller testis in the left inguinal region.
Bulky right sided iliac lymphadenopathy.
Large tumor replacing most of the right testis.
Normal left testis.

Case Discussion
Rarely do bilateral undescended testes present in middle age.
A strong association is recognized between undescended testes and a higher incidence of malignancy.
Locoregional nodes, lungs and liver are the typical sites for metastatic disease.
