Spontaneous subdural hematomas

Presentation

Headache for 5 days. No history of trauma. Taking ibuprofen. Gum bleeding.

Patient Data

Age: 30 years
Gender: Male

Thin subdural hematomas involving the falx cereberi, extending along the right tentorium cerebelli, as well as along the right lateral convexity. 

Diagram

Full blood examination demonstrated marked pancytopaenia. 

Case Discussion

This patient was diagnosed with acute promyelocytic leukemia (APML). Hematological malignancies are one of the causes of spontaneous subdural hematoma. Other risk factors include

  • hypertension
  • vascular malformations
  • dural tumor, e.g. metastases
  • infection
  • coagulopathy
