Presentation

Increasing nonspecific abdominal pain over the last 3 days. Constipation. Known chronic cholecystitis and chronic renal insufficiency.

Patient Data

Age: 75 years
Gender: Female
x-ray

An abdominal radiograph demonstrates air-fluid level within the small bowel.

An intraperitoneal catheter with tip projected at the right upper quadrant.

Because of known renal insufficiency, imaging is without IV contrast. 

Small bowel obstruction with visible gallbladder stone about 15 cm proximal to the ileocecal valve and air in the gallbladder.

A small volume of free fluid in the pelvis.
