What is the differential diagnosis of multiple bilateral lung masses?
Very broad. Generally speaking, it can be: neoplastic, inflammatory, infective and granulomatous.
What is the single best possible diagnosis in this case and why?
Hydatid cyst is highly likely given the multiplicity of the lesions and the undulating air-fluid level, what's known as "water-lily sign".
The CXR shows multiple, well-defined, large, rounded opacities in both hemithoraces. There is no calcification within these opacities. One of these appears to be a cavitating lesion in the right upper-mid zone, with somewhat thick-walled and smooth inner margin, showing an undulating/wavy air-fluid level.
No pleural effusion.
The visualised skeleton appears unremarkable.
