Left ventricular pseudoaneurysm

Case contributed by Maxim Stalkov
Diagnosis certain

Presentation

Chest pain post radiofrequency catheter ablation.

Patient Data

Age: 85 years
Gender: Female

Cardiac CT demonstrates left ventricular pseudoaneurysm. Myocardial defect is located in the lateral wall in close proximity to the mitral valve. Loculated hemopericardium and sliding hiatus hernia are also noted.

The four-chamber view clearly demonstrates the breach in the left ventricular wall.

On the 3D reconstruction, note the circumflex artery passing through the pseudoaneurysm.

Case Discussion

Pseudoaneurysms represent free wall rupture of the left ventricle contained by adherent pericardium. Postsurgical pseudoaneurysms are as common as post-infarction.
