Presentation

A heavy object fell over the right foot. Persistent pain.

Patient Data

Age: 45 years
Gender: Male

X-ray Right foot

Transverse simple fracture through the diaphysis of the second metatarsal bone with dorsal displacement of the distal fragment. Comminuted transverse fracture through the diaphysis of the third metatarsal bone, without displacement.

CT Right foot

Transverse simple fracture through the diaphysis of the second metatarsal bone with 5mm dorsal displacement of the distal fragment. Comminuted (3-part) transverse fracture through the diaphysis of the third metatarsal bone, without displacement.

The remainder bones are unremarkable, with normal alignment and no signs of fractures.

Case Discussion

This case reflects the typical appearance of fractures involving metatarsal bones when heavy objects fall over the dorsum of a foot: both fractures are at the same level in adjacent metatarsal bones, in the center of the foot, without other lateral bones displacement or fractures.
