Urethral strictures typically follow infection or trauma. In this case the 'trauma' was due to a geographically refined cultural practice of injecting paraffin into the penis to 'aid' enhancement.
The same process is performed - not by the medical estalishment - into female breasts to result in enhancement.
The size increase in the respective organs is due to a granulomatous foreign body reaction to the paraffin. A so called 'paraffinoma'. Complications are common - in this case a deformed penis with a anterior urethral stricture.
